Job Title

RN Home Health - Grand Rapids, MI (Local or Travel)

Assignment Details

• Assignment Length: 13 weeks

• Schedule: Monday-Friday, 8:00 AM-5:00 PM

• Hours: 40 hours per week

• On-Call: Every other week; triage nurse takes calls

• Weekend Coverage: Weekend RN available

• Productivity: 30 units

• EMR: HCHB (Homecare Homebase)

• Required Experience: HCHB, OASIS, and Home Health

• Guaranteed Hours: No guarantee

Requirements & Qualifications

• Experience: 2+ years of Home Health experience

• HCHB & OASIS: Required

• License: Active RN license in the practicing state

• CPR Certification: Required

• Driver's License: Required

• Transportation: Reliable transportation and valid auto insurance required

• CHPN Certification: Preferred

• Education: Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program required

• BSN: Preferred

Required Skills

• Homecare Homebase (HCHB): Required

• OASIS: Required

• Strong clinical judgment and patient assessment skills

• Excellent communication and documentation skills
