Home Health & Hospice Registered Nurse (RN)

Job Overview

The Home Health & Hospice Registered Nurse (RN) provides high-quality care to patients in their homes, ensuring their comfort and well-being through skilled nursing practices. This role entails working with a diverse population, including patients with various medical diagnoses and conditions.

Job Duties

• Deliver nursing care to home health and hospice patients, managing conditions such as Congestive Heart Failure (CHF), Diabetes, Renal Disease, and Wound Care.
• Administer IV therapy and manage drains and other medical devices.
• Perform comprehensive assessments and document findings in OASIS format as per compliance standards.
• Conduct admissions, recertifications, and resumptions, ensuring thorough documentation and patient management.
• Collaborate with a multidisciplinary team to create and implement individualized patient care plans.
• Adhere to strict infection control practices within home environments.
• Participate in on-call rotations as required.

Qualifications

• Active Registered Nurse (RN) license.
• Basic Life Support (BLS) certification required.
• Minimum of 1 year of recent home health experience within the last 2 years.
• Proficiency in OASIS documentation is a must.
• Experience in wound care and excellent infection control practices.
• Ability to manage a workload of 6 patient points per day, including admissions and follow-up visits.

Work Schedule

This position typically requires a commitment of 40 hours per week, with work hours scheduled from 8:00 AM to 4:30 PM, Monday through Friday. Nurses are expected to rotate weekend responsibilities and be flexible for on-call evening shifts.

Team Support

Nurses will work alongside a dedicated team which includes an Administrator, RN Coordinator, and office support supervisors who are available for assistance after hours.

Dress Code

All team members are expected to wear solid color scrubs that are clean and professional.
