Job Description:
Seeking an experienced Home Care RN to provide skilled nursing care to patients in their homes. Responsibilities include patient assessments, developing and implementing plans of care, wound care, medication administration, IV therapy, patient and family education, care coordination, and accurate clinical documentation.
Required Experience:
- Minimum 2 years of RN experience in a clinical care setting.
- Minimum 2 years of home health experience with a certified home care agency.
- Homecare Homebase software experience – Required.
- OASIS experience – Required.
Required Clinical Skills:
- Wound care, including wound VACs.
- Foley and suprapubic catheter insertion and care.
- PICC line care and IV therapy.
- Cardiac and respiratory assessment and patient education.
- Medication administration, venipuncture, and phlebotomy.
Required Qualifications:
- Active RN license for the state of New Jersey.
- Current BLS certification.
- Valid driver's license.
- Reliable transportation and automobile insurance.
- Associate Degree in Nursing or nursing diploma from an accredited program.
Preferred:
- 5 years of experience with certified home care agencies.
- BSN degree.
- CHPN certification.
- Epic experience.
Additional Requirements: Strong communication and organizational skills, ability to work independently, and willingness to travel locally between patient homes.
