Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

 

Location: Jacksonville, FL, 32216

Coverage Areas: Nassau & Duval County, FL

Weekly Gross Pay: Up to $2,547.00

Setting: Home Health

Schedule: Monday-Friday, 8:30 AM - 5:00 PM

Weekly Hours Guaranteed: 40 Hours

Productivity Expectation: 30 Points

On-Call/Weekend Requirement: Every Other Weekend

Mileage Reimbursement: Yes (between patient visits)

Dress Code: Any Solid Color Scrubs

Position Overview

  • We are seeking an experienced and compassionate Registered Nurse (RN) to join our Home Health team serving patients throughout Nassau and Duval County.
  • The ideal candidate will be responsible for delivering high-quality skilled nursing care, conducting comprehensive assessments, coordinating care plans, and ensuring compliance with home health regulations and documentation standards.
  • This role requires strong clinical judgment, OASIS expertise, proficiency with HCHB/PointCare, and the ability to work independently while collaborating effectively with physicians and interdisciplinary care teams.

Required Qualifications

  • Active Registered Nurse (RN) license.
  • Current CPR Certification.
  • Valid Driver's License.
  • Current vehicle insurance and access to a dependable vehicle.
  • Proficiency with HCHB/PointCare.
  • Strong OASIS assessment and documentation experience.
  • Home Health nursing experience preferred.
  • Ability to work independently in a fast-paced field environment.

  • Conduct accurate patient assessments, OASIS evaluations, and medication reconciliation while ensuring compliance with home health regulations and payer requirements.
  • Develop and manage individualized plans of care in collaboration with physicians and interdisciplinary healthcare teams.
  • Deliver high-quality skilled nursing services and oversee clinical care provided by LPN/LVNs through effective supervision and guidance.
  • Maintain timely, accurate, and compliant clinical documentation using HCHB/PointCare and agency-approved systems.
  • Coordinate patient care, physician communication, scheduling updates, and family education to achieve positive patient outcomes and service excellence.
