TravelNurseSource is working with Malone Healthcare to find a qualified Home Health RN in Newton, Massachusetts, 02458!
Key Responsibilities
Assess patient conditions and implement individualized care plans in accordance with organizational philosophy, policy, and community health practice standards
Provide ongoing education and support to patients and their families throughout the care continuum
Perform admission processes and fulfill case management responsibilities per established policies and standards of practice
Maintain thorough and timely electronic documentation and facilitate interdisciplinary communications
Travel to assigned patient cases within the service area
Participate in weekend rotations on a scheduled basis (approximately once every 6 weeks)
Required Qualifications
Active Registered Nurse (RN) license required
Minimum 1 year of experience in a Certified Home Health Agency setting (hospice, private duty, or standalone case management experience does not satisfy this requirement)
Minimum 1 year of Case Management experience
Ability to travel to patient cases as assigned
Preferred Experience / Skills
Familiarity with electronic health record (EHR) documentation systems
Strong interdisciplinary communication and collaboration skills
Experience with patient and family education in a home-based care environment
