Description

Key Responsibilities
  • Provide clinical services to patients ranging from birth to geriatric populations within their home environments
  • Assess patient conditions, develop and implement individualized care plans, and deliver ongoing education and support to patients and their families
  • Manage the admissions process and fulfill case management responsibilities in accordance with established policies and standards of practice
  • Maintain thorough and timely electronic documentation and facilitate interdisciplinary communications
  • Participate in weekend, holiday, and on-call rotations as needed by department (weekend rotation expected approximately once every six weeks)
Required Qualifications
  • Current Registered Nurse (RN) licensure in the state of Massachusetts
  • Minimum of 1 year of certified home care experience
  • Minimum of 1 year of case management experience
  • Reliable, independent means of transportation for conducting home visits
  • Willingness to work weekends, holidays, and on-call shifts in accordance with departmental rotation needs
Preferred Experience / Skills
  • Experience working with a broad patient population spanning pediatric through geriatric age groups
  • Familiarity with electronic health record (EHR) documentation systems
  • Strong interdisciplinary communication and collaboration skills
