License: Valid Registered Nurse license required
Certifications: BLS (AHA) required
Must-Have:
Recent home health experience
Proficiency in OASIS documentation
Experience in wound care
Excellent infection control practices in the home environment
Description:
The position involves providing care to home health and hospice patients.
Common diagnoses include CHF, diabetes, renal disease, wound care, and IV therapy management.
The nurse will handle admissions, re-evaluations, and resumptions for patients.
A typical day would involve 6 patient points, including admissions and follow-ups.
There is no additional utilization review required for this role.
Travel will be necessitated to visit patients in their homes.
There are CNAs available who assist in the role.
An administrator, RN coordinator, or office support supervisor will be available after hours as needed.
Weekend and holiday commitments are required, typically every 4th or 5th weekend and up to two holidays per year.
The nurse will participate in the call rotation, usually no more than one night of call per week.
All nurses will work in both home health and hospice settings.
The organization has a professional dress code with any solid color attire acceptable.
All clinicians must clock in and out on location.
If a clinician has worked previously, they must have a gap of 6 months before applying again.
