Description:
This position involves providing care for home health and hospice patients.
Common diagnoses include Congestive Heart Failure (CHF), Diabetes, Renal Disease, Wound Care, and IV therapy management.
It is essential to have experience with management of drains and other devices.
A typical day involves seeing 6 patients, which includes one admission and 3-4 follow-up visits.
Travel nurses are required to conduct Start of Care (SOC), Re-certifications, and Resumptions.
There will be nursing support available after hours.
CNA's are employed in the home health/hospice role for additional support.
Scheduling is managed by the office, with expectations to work 5 days per week, including rotating weekends.
Typically, the nurse will have one weekend on-call per month and one evening on-call per week.
Participation in the on-call rotation is required, with no more than one night of call per week.
All nurses will work with home health and hospice patients.
It is important to maintain a clean and professional appearance, with any solid color scrubs preferred.
Each clinician must clock in and out during their shifts.
