Home Health Registered Nurse (RN)

Job Description

We are seeking a dedicated Home Health Registered Nurse (RN) with OASIS experience to join our healthcare team. The ideal candidate will provide compassionate care to patients in their homes, ensuring high-quality treatment and support throughout their healing process.

Shift Information

  • Schedule: Monday - Friday, 8am - 5pm
  • Some weekend coverage required
  • Emergency on-call may be scheduled

Key Responsibilities

  • Conduct patient assessments and develop comprehensive care plans based on OASIS guidelines.
  • Provide nursing care and support to patients in their homes.
  • Manage a variable schedule while traveling to multiple patient locations.
  • Adjust care plans as necessary to accommodate changes in patient condition.

Requirements

  • OASIS experience is required.
  • Home Health and Hospice experience is essential.
  • Must possess a valid driver's license in good standing.
  • BLS certification is required.
  • Competency in using an electronic medical record (EMR) system, specifically HealthWyse (Careficient).
  • A minimum of 2 years of relevant nursing experience is preferred.

Holiday Coverage

Holiday coverage may be required.
