Hospice Registered Nurse (RN)

Job Overview

We are seeking an experienced Hospice Registered Nurse (RN) to join our dedicated team in providing high-quality care to patients in a home health setting. The ideal candidate will have a strong background in hospice care, ensuring compassionate and efficient service for patients and their families during a critical time.

Shift Details

This position is a day shift role, requiring a commitment to a 5x8 hour schedule from 08:00 to 16:30.

Key Responsibilities

Conduct comprehensive assessments of patients' needs and develop individualized care plans.
Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
Provide education and support to patients and their families regarding diagnosis and treatment options.
Collaborate with interdisciplinary teams to enhance patient care and services.
Document patient interactions and care provided according to established guidelines.
Adhere to all regulations and best practices associated with hospice care.

Qualifications

Must hold an active Registered Nurse (RN) license.
Previous experience in hospice care within a home health setting is required.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficient in electronic medical records (EMR) systems.

Additional Information

Local candidates are encouraged to apply. Mileage reimbursement will be provided at the IRS rate for travel associated with the role. This position does not require on-call duties.
