Description

Job Title: Home Health & Hospice Registered Nurse Profession: Registered Nurse Specialty: Home Health & Hospice Duration: 13 weeks Shift: Days + Call Hours per Shift: 8 hours Experience: 1 year License: Current Driver's License & Auto Insurance required Certifications: BLS Certification required Must-Have: Home Health & Hospice experience required Description: Department: Home Health/Hospice RN. No clinical involvement. Mileage is not paid separately; it is factored into the bill rate. Common diagnoses include Adults, Geriatrics, and Pediatrics. EMR system used is HomeCare HomeBase. Shifts are from 8 AM to 5 PM with an on-call rotation for evenings. Weekend coverage is required as part of the on-call rotation. Holiday coverage is also required. Solid color scrubs are the dress code. Initial assessment of client and family to determine home health needs. Completion of a physical assessment and history of current and previous illness. Regular re-evaluation of client nursing needs. Utilization of health assessment data to determine nursing diagnosis. Initiation of a plan of care with necessary revisions as client status and needs change. Development of care plans and establishment of goals based on diagnosis. Inclusion of client/family in the planning process. Administration of medications and treatments as prescribed by the physician. Provision of health care instructions to the client as appropriate per assessment and plan of care. Identification of discharge planning needs as part of care plan development. Implementation of discharge planning prior to client discharge. Acting as Case Manager when assigned and coordinating client care for assigned caseload. Preparation of clinical notes and updates to the primary physician as necessary and at least every sixty days. Communication with the physician regarding the client’s needs and reporting changes in condition. Obtaining and receiving physician orders as required. Ensuring arrangements for equipment and necessary items and services are available. Instructing, supervising, and evaluating home health aide care provided. Service area covers a 60-mile radius. Candidates may not hold part-time or full-time positions with any associated facilities within the past year. Required time off needs to be submitted upfront; no additional requests will be approved after submission. The facility can cancel up to 4 shifts per 13-week assignment or 3 shifts for shorter assignments. Charting system is through other computerized systems not listed. BSN is not required.
