Position Summary

The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes by performing assessments, coordinating care, educating patients and families, and ensuring quality outcomes through evidence-based nursing practices.

Job Responsibilities

• Provide skilled nursing care to patients in a home health setting.

• Perform comprehensive patient assessments and develop individualized care plans.

• Complete OASIS assessments and maintain accurate clinical documentation.

• Utilize Homecare Homebase (HCHB) software for patient records and documentation.

• Educate patients and caregivers regarding medications, treatments, and disease management.

• Monitor patient conditions and communicate changes to physicians and care teams.

• Collaborate with interdisciplinary healthcare teams to coordinate patient care.

• Maintain compliance with home health standards and documentation requirements.

• Participate in on-call responsibilities as needed based on branch and staffing needs.

Schedule & Assignment Details

• Full-time position available.

• Monday-Friday, 8:00 AM-5:00 PM.

• 40 hours per week.

• On-call required if requested.

• On-call requirements may change based on staff availability and branch needs.

• Guaranteed Hours: 0 Hours.

• Productivity Requirement: 30 Units.

Required Qualifications

• Active Registered Nurse (RN) license in the applicable state.

• Homecare Homebase (HCHB) software experience required.

• OASIS experience required.

• Current CPR certification required.

• Valid driver's license required.

• Must have reliable transportation and valid insurance.

• 1-2 years of Registered Nurse experience in a clinical care setting required.

• 2+ years of Home Health experience required.

Preferred Qualifications

• CHPN (Certified Hospice and Palliative Nurse) certification preferred.

• Bachelor of Science in Nursing (BSN) preferred.

Education Requirements

• Graduate of an accredited Associate Degree in Nursing (ADN) program or Nursing Diploma program required.

• Bachelor of Science in Nursing (BSN) preferred.
