Description

Key Responsibilities
  • Conduct OASIS admissions, all time points, and episode management across an assigned patient caseload of approximately 20-25 patients with an average of 4.5 visits per day
  • Perform coordination of care including physician communication and timely documentation in the EMR
  • Demonstrate clinical competencies in wound care, IV therapy, port access, lab draws, and chronic disease management
  • Travel within an assigned territory, with daily mileage ranging approximately 25-60 miles depending on census, territory, and staff availability; mileage reimbursed at the government-approved rate
  • Participate in on-call rotation approximately 2-3 times per month, with callback pay applicable if called back in during low-census unbillable hours
  • Maintain compliance with all facility policies, including applicable infection control and vaccination requirements
Required Qualifications
  • Active Wisconsin RN license or valid Compact RN license
  • AHA or ARC BLS certification required
  • Valid driver's license required
  • Minimum 2 years of home health nursing experience
  • Prior travel nursing experience required
  • OASIS charting experience required; experience with Epic Remote Client/Remote Care required
  • Must reside at least 50 miles from the facility to be eligible
Preferred Experience / Skills
  • OASIS certification is a plus
  • Strong time management skills with the ability to adapt to changing census and territory demands
  • Demonstrated resilience, accountability, and ability to manage change in a dynamic home health environment
  • Compassionate, dependable, and positive approach to patient care and team collaboration
