License: Active nursing license required. Certifications: CPR and BLS. Must-Have: Proficiency in HomeCare HomeBase and OASIS within the last 6 months. Description: This is a home health assignment. The clinician will be responsible for driving to patients' homes daily. Weekly meetings will occur at the clinic. A personal vehicle, valid driver's license, and auto insurance are required. The mileage radius for this position is 50-75 miles. Long drives between patients may be required. Understanding and acceptance of a patient schedule, regardless of distance, is essential. All documentation must be completed within 24 hours of the patient appointment. Incomplete documentation that affects billing and follow-up care may result in cancellation of the assignment. There are no guaranteed hours with this contract. The clinician must meet a weekly productivity point requirement of 30, reflecting approximately 40 hours of work per week. Overtime must be pre-approved. If patient load is insufficient, options may include floating to a nearby clinic for additional hours.
