Key Responsibilities
  • Provide skilled home health nursing care to patients in accordance with individualized care plans
  • Perform Start of Care (SOC), Resumption of Care (ROC), recertification, revisit, and OASIS discharge assessments
  • Complete accurate and timely clinical documentation using Homecare Homebase EMR and OASIS
  • Participate in on-call and phone triage rotation, covering weekday evenings (5:30 PM - 8:30 AM) approximately one to two weeks per month, with rare in-person home visits required
  • Meet productivity expectations of 30 points per week based on visit type (e.g., SOC/ROC = 2 pts, Recert = 1.5 pts, Revisit = 1 pt)
  • Deliver compassionate, patient-centered care with a strong focus on customer service excellence
Required Qualifications
  • Active RN licensure - must be in hand at time of submission
  • Minimum of 2 years of home health nursing experience
  • Homecare Homebase and OASIS experience required within the last year
  • Demonstrated reliability, dependability, and strong time management skills
  • Professional demeanor with a commitment to compassionate, high-quality patient care
Preferred Experience / Skills
  • Experience with Wellsky EMR
  • Wound care experience
  • Prior on-call or phone triage experience in a home health setting
