Key Responsibilities
Provide individualized, goal-oriented nursing care to patients in a home health setting using the nursing process
Carry out nursing care in accordance with physician orders
Serve as Case Manager when functioning as the lead discipline on a case
Coordinate and oversee patient care plans to support positive health outcomes
Required Qualifications
Active Registered Nurse (RN) license in the state of Vermont
Experience in home health or community-based nursing
Ability to apply the nursing process in a home care environment
Strong organizational and case management skills
Preferred Experience / Skills
Prior experience as a Case Manager in a home health or hospice setting
Familiarity with hospice, long-term care, or maternal-and-child health services
Experience serving diverse patient populations with varying financial circumstances
