Job Summary
This is a 25-week contract opportunity for an experienced Registered Nurse specializing in Home Health and Hospice care. The role involves providing comprehensive outpatient nursing services to patients within their homes in Newport, Vermont, and surrounding areas.
The successful candidate will manage a diverse caseload, delivering direct patient care, conducting thorough assessments, and developing individualized care plans. This position requires independent travel between patient residences, strong clinical judgment, and the ability to adapt to varying patient needs and schedules.
Key Responsibilities
- Provide skilled nursing care to home health and hospice patients in their residences.
- Conduct comprehensive patient assessments, including OASIS documentation, to determine patient needs and develop appropriate care plans.
- Administer medications, perform wound care, and implement physician orders.
- Monitor patient progress, identify changes in condition, and communicate effectively with physicians and the interdisciplinary team.
- Educate patients and their families on disease processes, medication management, and self-care techniques.
- Maintain accurate and timely electronic medical records using the HealthWyse (Careficient) EMR system.
- Collaborate with other healthcare professionals to ensure coordinated and holistic patient care.
- Manage a dynamic schedule, traveling efficiently between patient homes to provide scheduled visits.
Required Qualifications
- Active Registered Nurse (RN) license.
- Demonstrated experience in Home Health and Hospice settings.
- Proficiency in completing OASIS (Outcome and Assessment Information Set) assessments.
- Valid Driver's License in good standing.

Shift Details
- Shift Type: AM
- Hours: Monday - Friday, 8:00 AM - 5:00 PM (8-hour shifts)
- Days per week: 5
- Weekend Rotation: Some weekend coverage is required.
- Call Requirements: Emergency call may be scheduled.
- Contract Length: 25 weeks

Licensing Requirements
- Current Registered Nurse (RN) license in the state of Vermont.
- Basic Life Support (BLS) certification.
