This Home Health Registered Nurse role is based in Plymouth, Massachusetts, a picturesque coastal town widely recognized for its historic significance and welcoming New England charm. As a Home Health RN in this community, you will provide skilled nursing care to patients in their homes across Plymouth and surrounding areas. You will perform comprehensive assessments, manage ongoing clinical needs, coordinate care plans, and support patients and families through education and coaching to promote independence and optimal health outcomes. This setting offers meaningful, relationship-based care where you can see the direct impact of your work on patients’ daily lives.
Typical responsibilities include completing initial and follow-up assessments, developing and updating individualized plans of care, administering medications, monitoring vital signs, performing wound care and other skilled nursing procedures, and teaching patients and caregivers about disease management, medication regimens, and safety in the home. You will communicate regularly with physicians, therapists, social workers, and other team members to ensure coordinated, patient-centered care.
A typical day in this role includes visiting multiple patients in their homes, documenting care and progress using an electronic medical record system, and adjusting care plans based on clinical findings. You will organize your schedule to balance skilled visits, follow-up checks, and coordination tasks, while maintaining timely and accurate documentation. Caseloads and visit numbers are managed to support safe and effective care, with a focus on quality outcomes and patient satisfaction rather than high-volume throughput.
