Key Responsibilities
Conduct approximately 25-30 patient visits per week, including a mix of start-of-care and revisit encounters
Perform and complete OASIS assessments accurately and in a timely manner
Document all clinical activities and patient information using Epic EMR
Deliver skilled nursing care to home health patients in accordance with individualized care plans
Required Qualifications
Active Registered Nurse (RN) license in the state of Pennsylvania (or compact eligibility)
Minimum of 2 years of recent and relevant home health experience
OASIS experience required
Epic EMR experience required
Preferred Experience / Skills
Experience managing a high-volume caseload in a home health setting
Strong clinical assessment and documentation skills
