This is an in-person home health position, offering flexibility in how you structure your day. The position includes manageable caseloads and meaningful one-on-one care. What You'll Do: Perform comprehensive patient assessments. Develop and update individualized plans of care. Provide skilled nursing care and appropriate interventions. Monitor patient progress and report changes to the care team. Educate patients and families on medications, treatments, and care plans. Coordinate care with LPNs, therapists, caregivers, and providers. Complete timely and accurate documentation in EMR. Participate in on-call rotation.
