Description:
Provides, coordinates, and directs the provision of home nursing care according to physician orders based on agency policies and procedures.
Responsible for the competent application of the nursing process.
Oversees and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Requires travel to patients' homes for visits.
Must have reliable transportation and a current driver's license.
Must maintain agency-required liability insurance.
Contractor will manage a caseload of approximately 5-10 patients per day.
Hours may vary based on patient census.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Must be willing to travel up to 75 miles.
Mileage will be reimbursed at the IRS standard rate.
All documentation must be completed within 24 hours of patient appointments.
Delayed and incomplete documentation will affect follow-up patient care.
Must accept patients regardless of distance to previous appointments.
On-call rotations may be required.
Weekend rotation will vary per location.
All travelers must have used HomeCare HomeBase within the last 6 months or complete a retraining course.
Experience in home health or hospice is required.
Proficiency in OASIS is also necessary.
Adherence to productivity expectations is essential.
