Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum of one full year of recent adult home health experience License: Active nursing license required Certifications: CPR and BLS required Must-Have: Proficiency in Homecare Homebase (HCHB) and OASIS Description: This position involves providing home health services, including patient visits and attending periodic office meetings. The average daily caseload is 58 patients. The weekly productivity expectation is 30 points. Visit types, documentation, travel, and other activities have different point values. Contractors typically see 5 to 10 patients per day, totaling approximately 68 points. There are no guaranteed hours. If the patient census is low, contractors should contact the Account Manager about potentially floating to a nearby clinic for additional hours. Case types may include Start of Care, Wound Care, Follow-up visits, and other adult home health services. Traveling up to 75 miles from the assigned care center is required. Reliable transportation, a current driver’s license, and agency-required liability insurance are mandatory. Mileage is reimbursed at the standard rate. Travel between patient homes is reimbursable, but travel to the first patient and from the last patient is not billable. First-time travelers are not accepted; recent specialty experience is required. Contractors must be proficient in HCHB and have used it within the past six months. Clinicians with prior HCHB experience outside the six-month window may be considered, but they must complete retraining. Weekly schedules typically consist of Monday through Friday shifts. Clinicians must accept all scheduled patients, regardless of distance, and complete all documentation within 24 hours of each appointment. Non-compliance with documentation timelines may result in cancellations. It is understood that there are no guaranteed hours with this contract, and clinicians are expected to meet weekly productivity points. All overtime must be pre-approved. If fewer patients are assigned, clinicians will have the opportunity to float to a nearby clinic to pick up additional patients.
