Job Title: Registered Nurse Profession: Registered Nurse (RN) Specialty: Home Health Duration: 13 weeks Shift: 8:00 AM - 5:30 PM Hours per Shift: 8 Experience: Minimum years of experience in Home Health required License: Valid nursing license required Certifications: BLS required Must-Have: Home Health experience Valid driver's license and car insurance OASIS and start of care experience preferred Description: Prescribes, coordinates, and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises CNA and coordinates care with other healthcare workers as assigned. Provides comprehensive initial assessments to all clients and on subsequent visits. Continually assesses physical, psychosocial, environmental, and community resources available. Utilizes the principles of growth and development over the lifespan. Initiates the Plan of Care and re-evaluates the need for care. Modifies the Plan of Care as ordered or needed. Establishes discharge plans with patients, physicians, other disciplines, and caregivers. Completes OASIS data. Provides patient care using current federal and state regulations. Assesses educational needs of patients, families, and caregivers. Implements appropriate teaching strategies. Completes documentation according to regulatory guidelines. Promotes a clean and safe environment of care. Utilizes safe error prevention habits. Provides the highest standard of privacy and confidentiality in all matters involving patients and coworkers. Abides by the Standards of Conduct.
