NEW CLIENT
Start Date: 9/28/26 or soonest available.
Number of positions open: 1 part-time RN Case Manager, increasing to full-time position.
Schedule request: Availability Monday to Friday and occasionally weekend depending on client needs and visit days. Day hours. Flexibility.  
Productivity Expectations: Varies from week to week. Approximately 8-15 clients per week currently. Increasing up to 15-30 clients per week based on census and new admissions.
Territory: Minneapolis/St Paul metropolitan area including counties of Anoka, Ramsey, Hennepin, Dakota, Washington and Scott.
EMR: ALORA Plus
Requirements: Competent in current OASIS for Start of Care, Recertification, Resumption of Care, Change in Condition, and Transfer/Discharge. Competent in Medicare requirements.  Prior RN Case Management experience in homecare setting. Skilled in wound care, medication management, and symptom management for multiple comorbid diagnosis (list is not all inclusive of skills). Team meetings as scheduled by Management. Involvement as needed in Quality Improvement Projects.
Mile rate: $0.65 per mile. (negotiable)
