Description: Prescribes, coordinates, and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises Certified Nursing Assistants and coordinates care with other health care workers as assigned. Provides comprehensive initial assessments to all clients and continuously assesses physical, psychosocial, environmental, and community resources available. Utilizes principles of growth and development over the lifespan. Initiates Plan of Care, re-evaluates the need for care, and modifies Plan of Care as ordered or needed. Establishes discharge plans with patients, physicians, other disciplines, and caregivers. Completes OASIS data. Provides patient care using current federal and state regulations. Assesses educational needs of patients, families, and caregivers, and implements appropriate teaching strategies. Completes documentation according to regulatory guidelines. Promotes a clean and safe environment of care using SAFE error prevention habits.
