Responsibilities include performing initial in-home pre-evaluations of patients that assess health status, strengths, care needs, and preferences. The role includes reviewing consumer medical documentation and health referral forms as relevant to cases. Evaluation data will be entered into an electronic form and transmitted as required. It is also necessary to document concerns, conflicting information, and issues that arise during the evaluation process. Emphasis on continuity of care is essential to reduce fragmentation, duplication, and gaps in treatment plans. Discussion with consumers regarding healthcare options, supports, service vendor options, and waiver options will be part of the role. Communication between the Call Center and eligible consumers for enrollment into managed care health plans is required. Enhancing communication and collaborative relationships with interdisciplinary care team members is vital for improved care coordination and service delivery. Quality review data and required documentation must be collected for outcome measurements and case notes recorded. Identification of opportunities for health promotion and illness prevention is expected. Maintain comprehensive knowledge of community resources, payor requirements, and services for the target population. All established standards for this position as outlined in the performance criteria and bonus template must be met. Additional duties may be assigned by the Regional Nurse Manager or other project management as required.
