Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and OASIS Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Home visits are required along with periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Contractor will be assigned between 5-10 patients per day, which equates to roughly 6-8 points. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Traveling up to 75 miles from the home clinic is necessary. Mileage is reimbursed at the IRS standard rate. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care will result in cancellation of assignments. Clinician must hit weekly productivity points of 30, which equates to 40 hours per week. Open to on-call rotations with availability for weekend shifts every 4-5 weeks. Local candidates are accepted at the same rate. Must be open to traveling to a nearby clinic if patient census is low. Must confirm that they are comfortable with the mileage radius and driving lengths. Declining patients may result in a cancellation of assignments. All applicants must have recent specialty experience, as first-timers are not accepted for this role.
