Description:
The position requires a Registered Nurse to provide home health services.
The nurse will be responsible for visiting patients in their homes.
The average daily caseload is 5 to 8 patients.
The weekly productivity expectation is 30 points.
Visits include a variety of case types such as Start of Care, Wound Care, and Follow-up visits.
Documentation must be completed within 24 hours of each patient appointment.
Travel may require driving up to 75 miles between patient homes.
Nurses must accept every patient on their schedule.
On-call duties will include weekends and some evenings as needed.
No guaranteed hours are provided, and productivity points must be met.
Candidates should be open to floating to other nearby clinics if necessary.
