Description:
Provides, coordinates, and directs the provision of home nursing care according to physician's orders.
Care is based on agency policies and procedures through the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Home visits and periodic office meetings are required.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day is 5-8.
Contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Requires a commitment to traveling up to 75 miles from the assigned care center.
The average weekly mileage may be approximately 500 miles.
Expected weekly productivity points are 30, which equates to a full-time schedule.
All documentation must be completed within 24 hours of the appointment.
Delayed documentation that impacts billing and follow-up patient care will result in a cancellation of the assignment.
The clinician must accept every patient on their schedule regardless of distance from previous appointments.
All travelers must comply with the proficiency requirements for HCHB and OASIS as noted.
This position requires flexibility with on-call rotations and possible weekend work.
