Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in OASIS and HomeCare HomeBase EMR Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders. The care is based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses or Licensed Vocational Nurses. Home visits are required. Periodic office meetings are also required. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. A contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points. There are no guaranteed hours. If there is a situation where the contractor is unable to meet financial needs due to low patient census, they should discuss floating to a nearby clinic to pick up hours. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-timers are not accepted; recent specialty experience is required. Weekend requirements vary by location and profession. On-call requirements will vary by location and profession. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS Standard rate. The commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. Pending licenses are not accepted; a license must be in hand. All candidates must be proficient in HomeCare HomeBase and have used it within the last six months. Alternatively, if used outside of the last six months, the candidate must complete a retraining course during compliance. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care may also result in a cancel. The expectations are that the clinician will accept every patient on their schedule, regardless of distance from previous appointments. Declining patients may result in a cancel. All overtime must be pre-approved. If not receiving enough patients weekly to support financial needs, the clinician will have the opportunity to float to another nearby clinic to pick up patients.
