Bilingual Spanish-English proficiency is strongly preferred. Candidates must have two (2) years of experience in a home setting within the last ten (10) years. This experience may include working in the field for a certified home health agency, licensed home care service agency, or managed long-term care. Conducting assessments similar to UAS, such as OASIS, is also required. Providing primary care to homebound individuals and managing care for those receiving home and community-based services, including hospice and assisted living, is necessary. Home care experience as a paraprofessional, such as a home health aide or licensed practical nurse, caring for a minimum of two patients per week is preferred. In addition to the required qualifications, preferred candidates will have experience conducting assessments in UAS-NY, particularly in the last 365 days. Experience with a geriatric population and managing their chronic diseases in the home setting is beneficial. Candidates will perform initial in-home pre-evaluations of patients, evaluate their health status, strengths, care needs, and preferences. They will guide the development of individualized long-term care service plans. Candidates will review consumer medical documentation and enter evaluation data into electronic forms as required. Documenting concerns, conflicting information, and any issues that surface during the evaluation process is essential. Continuity of care is emphasized to reduce fragmentation, duplication, and gaps in treatment plans. Candidates will discuss health care options, needed supports, service vendor options, and waiver options with consumers. Initiating communication with consumers for enrollment into managed care health plans will also be part of the role. Enhancing communication and relationships with interdisciplinary care team members is necessary to improve care coordination and facilitate service delivery. Collecting quality review data and documenting required information to support outcome measurements is expected. Candidates must maintain a comprehensive knowledge of community resources and network services for the target population. They will meet all performance standards established for this position. Performing other duties as assigned by the regional nurse manager or project management may also be required. All candidates must be prepared for hybrid tele and in-field assessments. They must also provide required personal information such as aliases used, current and previous addresses, and current phone numbers and emails.
