License: Current RN Licensure in Client Certifications: Basic Life Support (BLS) Must-Have: Recent experience conducting comprehensive assessments of patients Description: Urgent need for Registered Nurses. Candidates must have current RN licensure in Client. A minimum of two years of experience in a home health setting within the last fifteen years is required. Candidates must have recent experience conducting comprehensive assessments of patients. Candidates will conduct hybrid telehealth and in-field assessments. Ability to review health status questions and make evaluations based on criteria for enrollment. Familiarity with technology in the workplace is necessary. Knowledge of general professional nursing theories, principles, and practices is essential. Understanding of state laws and regulations governing nursing practices is required. Excellent organizational, interpersonal, written, and verbal communication skills are crucial. Candidates should communicate and work effectively with multi-generational consumers. Demonstrating sensitivity to the concerns of others is important. Ability to perform comfortably in a fast-paced, deadline-oriented work environment is necessary. Candidates should be able to manage multiple complex tasks simultaneously. Strong ability to work independently is a must. Primary responsibilities include performing initial in-home pre-evaluations of patients. Candidates will evaluate health status, strengths, care needs, and preferences to guide long-term care service plans. Review of consumer medical documentation and health referral forms will be required. Evaluation data must be entered into electronic forms and transmitted as necessary. Candidates should document any concerns or issues that arise during evaluations. Emphasis must be placed on continuity of care to reduce fragmentation and gaps in treatment plans. Discussion with consumers regarding healthcare options, support needs, and service vendors is required. Initiation of communication between consumers for enrollment into a managed care health plan is essential. Enhancing communication and collaborative relationships with interdisciplinary care team members will improve care coordination. Collecting quality review data and any required documentation is necessary for outcome measurements. Maintaining a comprehensive knowledge of community resources and network services is important for the target population. Candidates should meet all performance standards established for this position. Additional duties may be assigned by the Regional Nurse Manager or project management.
