License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: Bilingual Spanish-English proficiency strongly preferred Description: We have an urgent need for Home Health Registered Nurses. Candidates must possess current RN licensure in Client. A minimum of two years of experience in a home health setting within the last fifteen years is required. Recent experience conducting comprehensive assessments of patients is essential. Nurses will perform initial in-home evaluations of patients. This includes evaluating health status, strengths, care needs, and preferences. These assessments will guide the development of individualized long-term care service plans. The role involves reviewing consumer medical documentation and health referral forms as necessary. Candidates will enter evaluation data into electronic forms and transmit it as required. Documenting concerns and issues that arise during evaluations is part of the job. Continuity of care must be emphasized to reduce fragmentation and gaps in treatment plans. Nurses will discuss various healthcare options with consumers and identify needed supports. The role requires initiating communication for enrollment into managed care health plans. Enhancing communication with interdisciplinary care team members is expected. Candidates will collect quality review data to support outcome measurements. Maintaining knowledge of community resources and payer requirements is essential. All candidates must be prepared to conduct both in-field and tele-assessments. Additional duties may be assigned by the Regional Nurse Manager or project management.
