Enter evaluation data into electronic forms and transmit as required. Document any concerns or issues that surface during evaluations. Emphasize continuity of care to reduce fragmentation and gaps in treatment. Discuss available healthcare options, supports needed, and service vendor options with consumers. Initiate communication for enrollment into managed care health plans. Enhance collaborative relationships with interdisciplinary care team members to improve service delivery. Collect quality review data and documentation to support outcome measurements. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources and services available to the target population. Meet all performance standards established for this position. Perform other duties as may be assigned by management.
