Description:
Provides, coordinates, and directs the provision of home nursing care according to physician's orders.
Care is based on agency policies and procedures through the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Travel is required for home visits and periodic office meetings.
Expect to manage an average of 5-8 patients per day.
Each contractor will be assigned between 5-10 patients per day.
Patient visits equate to roughly 6-8 points.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Must be willing to travel up to 75 miles from the assigned care center.
Mileage will be reimbursed at the IRS standard rate.
Includes weekend rotation every 4-5 weeks.
On-call duty is required, typically a 7-day stretch occurring every 4-5 weeks.
Documentation must be completed within 24 hours of the appointment.
Clinicians are expected to accept every patient on their schedule, regardless of distance.
No guaranteed hours with this contract; clinicians must hit a weekly productivity expectation of 30 points.
If insufficient patients are assigned, floating to another nearby clinic may be necessary.
