Home Health Registered Nurse

This role is for a dedicated Home Health Registered Nurse (RN) based in Rutland, Vermont. The successful candidate will provide comprehensive, patient-centered nursing care to individuals in their homes, utilizing the full scope of the nursing process to assess, plan, implement, and evaluate care.

As a Case Manager, you will be responsible for managing a diverse caseload, coordinating care with patients, caregivers, and an interdisciplinary team, and ensuring adherence to evidence-based practices and regulatory standards. A key aspect of this role involves accurate and timely OASIS documentation and maintaining proficiency in high-tech nursing skills.

Key Responsibilities
  • Conduct comprehensive patient and caregiver assessments to develop individualized plans of care.
  • Serve as a Case Manager, actively managing patient caseloads and visit schedules in accordance with agency productivity standards.
  • Complete OASIS documentation accurately and timely, staying current with all regulatory requirements.
  • Implement medical plans of care and nursing interventions, including advanced skills such as IV therapy, wound VAC management, and complex wound care.
  • Collaborate effectively with Licensed Practical Nurses (LPNs), delegating tasks and reviewing documentation to ensure continuity of care.
  • Evaluate the effectiveness of the plan of care, patient responses, and make necessary adjustments.
  • Maintain accurate, complete, and timely documentation of all nursing care provided.
  • Provide thorough patient and caregiver education on medications, treatments, discharge instructions, and reportable conditions.
  • Identify and coordinate interdisciplinary patient needs, including physical therapy and social work, communicating information promptly.
  • Complete all required Agency, State, and Federal forms, such as HHABN and Medicare Notice of Non-Coverage.
  • Coordinate safe patient discharges and transfers, ensuring all necessary paperwork and information are processed.
  • Collaborate with physicians, providers, and other healthcare team members to support the recommended plan of care.
  • Participate in monthly peer review audits and facilitate case conferences with the healthcare team.
  • Proactively advocate for patient care issues to ensure sensitive and appropriate care for each patient and family.
  • Adhere to agency protocols for Electronic Medical Record (EMR) processes, including visit completion and follow-up notes.
  • Assess patient needs for Home Care Aide (HCA) services, develop coordinated plans of care, and provide ongoing supervision.
  • Transmit patient incident reports (e.g., infections, injuries) to the Quality Improvement Specialist in a timely manner.
