Home Health Registered Nurse (RN)
Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to join a nonprofit home health agency serving the community of Mesa County, Colorado. This role focuses on providing skilled, patient-centered nursing care directly in patients' homes, fostering meaningful one-on-one interactions and contributing to improved patient outcomes.
This in-person, community-based position offers a unique opportunity for nurses who thrive in an autonomous environment. You will manage a structured caseload with the flexibility to organize your daily workflow, ensuring comprehensive care delivery while maintaining a healthy work-life balance. The role emphasizes independent visits within patient homes, supported by a collaborative and interdisciplinary team.
Key Responsibilities
- Perform comprehensive patient assessments to identify needs and develop individualized plans of care.
- Develop, implement, and regularly update patient-specific plans of care in collaboration with the healthcare team.
- Provide skilled nursing care, including medication administration, wound care, IV therapy, and other appropriate interventions.
- Monitor patient progress, evaluate the effectiveness of interventions, and report any significant changes to the care team and providers.
- Educate patients and their families on disease management, medications, treatment plans, and self-care techniques.
- Coordinate care effectively with LPNs, therapists, caregivers, and other healthcare providers to ensure continuity of care.
- Complete timely, accurate, and thorough documentation of all patient encounters and care activities within the Electronic Medical Record (EMR) system.
- Participate in the agency's on-call rotation as scheduled.
Required Qualifications
- Valid driver's license, reliable transportation, and current auto insurance.
Preferred Qualifications
- Minimum of one (1) year of experience in a home health setting.
- Familiarity and comfort with Homecare Homebase (HCHB) EMR system.

- Ability to work independently and effectively manage a structured caseload.
- Excellent professional communication skills, both verbal and written, with patients, families, and team members.
- Proficiency and comfort with electronic medical record (EMR) systems for documentation.

- Ability to collaborate effectively within an interdisciplinary team.
Shift Details
- Shift Type: Day, Flexible
- Shift Duration: 8 hours per day
- Days Per Week: 5 days (Monday - Friday)
- Call Requirements: Participation in an on-call rotation is required.
- Weekend Rotation: Not specified.
- Assignment Length: 13-week contract.

Licensing Requirements
- Active Registered Nurse (RN) License in the State of Colorado or an active Nurse Licensure Compact (NLC) license.
