Requirements: Current state license as a Registered Nurse; Homecare Homebase Software Experience; OASIS Experience; Proof of current CPR certification; 1-2 years experience as a Registered Nurse in a clinical care setting; 2+ years Home health experience; Must have reliable transportation and insurance.





Responsibilities:

  • Assess home health patients in stable and emergent situations to identify needs and develop plans of care.
  • Coordinate care with interdisciplinary teams, patients, families, and referring agencies.
  • Educate patients and families on care, medications, and safety measures.
  • Supervise and provide directions to home health staff to ensure quality care.
  • Maintain accurate documentation of patient visits, treatments, and progress.
