Job Opportunity – Registered Nurse (RN) – Home Health – Colorado

Start Date: September 8, 2026

Location: Mesa County, Colorado

Compensation: Details provided to qualified applicants

Schedule: Monday through Friday day schedule with on-call rotation. Flexible day-to-day workflows with independent patient visits in the community.

Requirements:

  • Strong assessment and critical thinking skills
  • Ability to work independently and manage a structured caseload
  • Professional communication skills with patients, families, and care team members
  • Comfort with clinical documentation and electronic medical record systems

Responsibilities:

  • Perform comprehensive patient assessments in home settings
  • Develop and update individualized plans of care
  • Provide skilled nursing care and appropriate clinical interventions
  • Monitor patient progress and communicate changes to the interdisciplinary care team
  • Educate patients and families on medications, treatments, and self-care strategies
  • Coordinate care with LPNs, therapists, caregivers, and healthcare providers
  • Complete timely and accurate documentation in EMR
  • Participate in on-call rotation to support continuity of care
