Schedule: Monday through Friday day schedule with on-call rotation. Flexible day-to-day workflows with independent patient visits in the community.

Requirements:
- Active Registered Nurse (RN) License in the State of Colorado or active NLC compact license
- Valid driver's license, reliable transportation, and current auto insurance
- Strong assessment and critical thinking skills
- Ability to work independently and manage a structured caseload
- Professional communication skills with patients, families, and care team members
- Comfort with clinical documentation and electronic medical record systems
Responsibilities:
- Perform comprehensive patient assessments in home settings
- Develop and update individualized plans of care
- Provide skilled nursing care and appropriate clinical interventions
- Monitor patient progress and communicate changes to the interdisciplinary care team
- Educate patients and families on medications, treatments, and self-care strategies
- Coordinate care with LPNs, therapists, caregivers, and healthcare providers
- Complete timely and accurate documentation in EMR
- Participate in on-call rotation to support continuity of care
