Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: 260 days Shift: Day Hours per Shift: 12 Experience: Two years of experience in a home health setting within the last ten years License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: - Two years of experience in a home setting within the last ten years. - Experience conducting comprehensive assessments of patients. - Experience working in field settings for a certified home health agency, licensed home care service agency, or managed long-term care. - Experience providing primary care to homebound individuals and managing care for those receiving home and community-based services. - Experience caring for a minimum of two patients per week. Description: Our organization is currently seeking a Home Health Registered Nurse. The candidate must have at least two years of experience in a home healthcare setting within the last decade. Experience conducting comprehensive assessments of patients is required. Candidates should demonstrate knowledge of the theory, principles, and practices of professional nursing. Applicants must also possess excellent organizational, interpersonal, written, and verbal communication skills. The role includes performing initial in-home evaluations of patients to assess health status, strengths, care needs, and preferences. Candidates will review medical documentation and health referral forms as relevant to each case. Data from evaluations must be entered into electronic forms and transmitted as required. The role emphasizes continuity of care, reducing fragmentation and gaps in treatment plans. Candidates will discuss healthcare options, supports needed, service vendor options, and waiver options with consumers. Communication with the call center and eligible consumers will be initiated for enrollment into managed care health plans. A comprehensive understanding of community resources, payor requirements, and network services is necessary.
