TravelNurseSource is working with Malone Healthcare to find a qualified Home Health RN in Buffalo, New York, 14201!
Key Responsibilities- Provide, coordinate, and direct home nursing care according to physician orders, agency policies, and procedures through competent application of the nursing process
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Conduct home visits covering an assigned territory within Erie County, NY; expected to travel up to 75 miles from the assigned care center
- Manage a caseload of approximately 5-10 patients per day, with an expected productivity target of 30 points per week (approximately 40 hours)
- Complete all patient documentation within 24 hours of each appointment using HomeCare HomeBase (HCHB) EMR; OASIS proficiency required
- Attend periodic office/clinic meetings; must maintain reliable transportation, a valid driver's license, and required auto liability insurance
Required Qualifications- Active, unencumbered RN license in hand - no pending licenses accepted
- Minimum 1 full year of recent Adult Home Health experience (first-time travelers not accepted; recent specialty experience required)
- CPR/BLS certification required
- Proficiency in HomeCare HomeBase (HCHB) EMR used within the last 6 months (if outside 6 months, a retraining course must be completed during compliance)
- OASIS proficiency required, used within the last 6 months (if outside 6 months, a retraining course must be completed during compliance)
- Must be willing and able to travel up to 75 miles from the assigned care center; must be open to on-call rotations and accepting all scheduled patients regardless of distance from previous appointment
- Local candidates are welcome and accepted at the same rate
Preferred Experience / Skills- Experience with a variety of case types including Start of Care, Wound Care, and Follow-Up Visits
- Familiarity with point-based productivity systems in a home health environment
- Comfort with extended driving between patient homes and managing a high-volume daily caseload
