Key Responsibilities

• Provide skilled nursing care to patients within an assigned home health region, with approximately 20% of visits conducted in assisted living facilities
• Participate in a required daily conference call at 8:15 AM
• Complete accurate and timely clinical documentation using Wellsky/OASIS
• Manage a patient caseload across a defined geographic region, requiring reliable personal transportation
• Adhere to professional dress standards including clean scrubs, closed-toe shoes, and limited facial jewelry

Required Qualifications

• Active Registered Nurse (RN) license
• Minimum of one year of recent experience as a Registered Nurse
• Minimum of six months of recent experience in a Home Health RN setting (private duty experience will not be considered)
• BLS and First Aid certification required (AHA or ARC accepted)
• Proficiency or ability to learn Wellsky/OASIS charting platform

Preferred Experience / Skills

• Experience working in or alongside assisted living facility environments
• Familiarity with OASIS documentation and home health regulatory requirements
• Strong time management and independent patient caseload management skills
