Job Title: RN Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 Experience: 1 year Home Health experience required License: Driver's License Certifications: BLS Must-Have: Homecare Homebase experience required Description: Home Health position available for a registered nurse. Teammates are required to complete six visits a day, including routine visits, discharge, reassessment, and recertification. Patients typically present with chronic diseases, wound care, IV needs, and more. Travel will be required, averaging 60-80 miles a day, with mileage reimbursed weekly. The areas of travel include several nearby towns. The ability to manage a caseload of 25-30 patients is essential. Skills needed include wound care, assessment, disease teaching, and experience with OASIS and case management. A company phone will be provided for documentation and communication. No company car is provided. Collaboration with departmental leadership and peers is expected. Weekend rotations are not required unless desired. Travelers are expected to work 75% of holidays. The departmental culture emphasizes teamwork. Visits will be scheduled by the nurse the day before. Cancelled appointments will not count towards visit equivalents. A focus on patient care and effective communication is critical.
