Conduct regular home visits based on patient needs. Attend periodic office meetings. Average daily caseload is 5 to 8 patients. Weekly productivity expectation is 30 points. Visit types, documentation, travel, and other activities have varying point values. Contractors typically see 5 to 10 patients per day, totaling approximately 6 to 8 points. There are no guaranteed hours. If patient census is low, contact the Account Manager about floating to a nearby clinic for additional hours. Case types may include: Start of Care, Wound Care, Follow-up visits, and other adult home health or hospice services. Candidates must have experience in home health. Candidates will be responsible for driving to patient homes daily. Reliable transportation is required. A current driver's license and agency-required liability insurance are mandatory. Mileage is reimbursed at the IRS standard rate. Travel between patient homes is reimbursable. Travel to the first patient and from the last patient is not billable. Signing and completing all documentation within 24 hours of the appointment are crucial for billing and follow-up patient care. Declining patients may result in a cancellation. All overtime must be pre-approved. Clinicians must be willing to travel up to 75 miles from the assigned care center. Clinicians should be open to on-call rotations. Local candidates are accepted at the same rate.
