Registered Nurse – Home Health

Assignment Details

• Shift: 5x8 Days, 8:30 AM – 5:00 PM
• Weekends: Alternating weekends
• Contract Length: 13 weeks
• Guaranteed Hours: 40 hours per week
• Start Date: 2 weeks from offer

Requirements

• Minimum 2 years of Home Health experience.
• Homecare Homebase (HCHB) and OASIS experience within the last year is mandatory.
• Active license must be in hand at the time of submission.
• WellSky EMR experience is a plus.
• Wound care experience is a plus.
• Experience with on-call duties and phone triage.
• Reliable, dependable, and professional.
• Excellent time management skills.
• Strong customer service skills.
• Compassionate and caring attitude.

On-Call Schedule and Expectations

• On-call coverage is required for 1–2 weeks per month.
• Weekday hours: 5:30 PM – 8:30 AM.
• Responsibilities primarily involve phone triage.
• Home visits during on-call hours are rarely required.

Productivity Requirements

- Full-time Home Health RNs must meet 30 points per week.
