Description:
Required licensure as a current RN in Client.
Two years of experience in a home setting within the last ten years, which may include working for a certified home health agency, licensed home care service agency, or managed long-term care.
Conducting assessments that resemble the UAS, such as OASIS.
Providing primary care to homebound individuals.
Care management for individuals receiving home and community-based services, including hospice and assisted living.
Experience as a paraprofessional or licensed practical nurse caring for a minimum of 2 patients per week is preferred.
Additional preferred experience includes conducting assessments in UAS-NY within the last 365 days.
Experience managing chronic diseases in a geriatric population within a home setting is also preferred.
Examples of experience that may be counted toward preferred qualifications but do not satisfy the required qualifications on their own include home care experience with only one patient, case management limited to utilization reviews for a health plan, and assessments conducted solely via telephone.
Candidates must be prepared to conduct hybrid tele and in-field assessments.
Candidates must not have ongoing assignments for UAS assessments elsewhere.
Ability to review health status questions and make evaluations and determinations based on UAS criteria for enrollment.
Ability to work with technology in the workplace.
Knowledge of nursing theory, principles, and practices.
Understanding of laws and regulations governing nursing practices.
Excellent organizational, interpersonal, written, and verbal communication skills.
Ability to communicate effectively with a diverse population.
Ability to demonstrate sensitivity to the concerns of others.

Job responsibilities include performing initial in-home pre-evaluations of patients, reviewing medical documentation and health referral forms, entering evaluation data into an electronic form, documenting concerns, and discussing healthcare options with consumers.
The role also involves enhancing communication and collaboration with interdisciplinary team members, collecting quality review data, identifying opportunities for health promotion and illness prevention, and maintaining knowledge of community resources and payor requirements.
