Candidates will perform initial in-home pre-evaluations of patients, evaluating health status, strengths, care needs, and preferences. They will guide the development of individualized long-term care service plans. The role involves reviewing medical documentation and/or health referral forms relevant to the case. Evaluation data must be entered into an electronic evaluation form and transmitted as required. Candidates will document concerns, conflicting information, and other issues that arise during the evaluation process. Emphasis is placed on continuity of care to reduce or eliminate fragmentation, duplication, and gaps in treatment plans. Professionals will discuss health care options, needed supports, service vendor options, and waiver options with consumers. Initiating communication between the designated center and eligible consumers for enrollment into a managed care health plan is also required. Enhanced communication and collaborative relationships with interdisciplinary care team members are essential to improve care coordination and facilitate service delivery. Quality review data must be collected and any required documentation recorded in the case notes. Candidates will identify opportunities for health promotion and illness prevention. A comprehensive working knowledge of community resources, payor requirements, and network services is necessary for the target population. All standards established for the position must be met as outlined in the corresponding annual performance criteria and bonus template. Additional duties may be assigned by the Regional Nurse Manager or project management.
