Initiate communication with eligible consumers for enrollment into managed care. Enhance communication and collaboration with interdisciplinary care team members. Collect quality review data and document case notes accurately. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources and payor requirements. Meet all standards established for the position as outlined in performance criteria. Perform other duties as assigned by the Regional Nurse Manager or project management.
