Candidates must have a current RN licensure in Client. A minimum of two years of experience in a home setting within the last fifteen years is required. Applicants should have recent experience conducting comprehensive assessments of patients. Nurses are expected to review health status questions and make evaluations based on UAS criteria for enrollment. Candidates must have the ability to work with technology in the workplace. Knowledge of the theory, principles, and practices of general professional nursing is essential. Understanding state and agency laws and regulations governing nursing practices is required. Excellent organizational, interpersonal, written, and verbal communication skills are necessary. Candidates should be able to effectively communicate and work with multi-generational consumers. Sensitivity to the concerns of others is vital for this role. The ability to perform comfortably in a fast-paced, deadline-oriented work environment is crucial. Candidates must demonstrate the capacity to successfully execute many complex tasks simultaneously. Working independently is a strong expectation for this role. Responsibilities include performing initial in-home pre-evaluations of patients, documenting all relevant health status information, and entering evaluation data into electronic forms. Nurses will emphasize continuity of care to reduce fragmentation and gaps in treatment plans. Communication with consumers regarding healthcare options and service vendors is necessary. Candidates must maintain awareness of community resources and payor requirements for the target population. Compliance with all performance standards for this position is required. Additional duties may be assigned by the Regional Nurse Manager or project management. All candidates must have the impression they will be conducting in-field assessments.
