Description

Job Title: RN Profession: Registered Nurse Specialty: Home Health Duration: Part-time Shift: 2x3 days Hours per Shift: Approx. 3 face-to-face visits per week Experience: Two (2) years of experience in a home setting within the last ten (10) years. License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: - Recent experience conducting comprehensive assessments of patients. - Experience in a home setting for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. - Experience caring for a minimum of 2 patients per week. Description: This position involves part-time work with approximate three face-to-face visits each week. All candidates must have two years of experience in a home care setting within the last ten years, which includes experience working in the field for a certified home health agency or similar. Candidates must have conducted assessments similar to OASIS and must have provided primary care to homebound persons. Experience caring for a geriatric population and managing their chronic diseases is preferred. Candidates will perform initial in-home evaluations of patients, including assessments of health status, strengths, care needs, and preferences. The role involves reviewing medical documentation and entering evaluation data into electronic forms for transmission as required. Documenting concerns and conflicting information during the evaluation process is essential. Candidates must promote continuity of care and communicate with consumers about healthcare options and needed services. Enhancing communication and collaboration with interdisciplinary care team members is crucial to improve care coordination. Identification of health promotion and illness prevention opportunities is also a responsibility in this role. Candidates should maintain comprehensive knowledge of community resources and network services for the target population. All candidates will be evaluated based on established performance criteria. Other duties may be assigned by the Regional Nurse Manager or other project management staff.
